UNCLAIMED BENEFITS CLAIMS PROCEDURE

In order to process payment from the Unclaimed Benefits Fund, Alexander Forbes will require the following information :

  •   Original Unclaimed Benefits claim form

  •   A certified copy of the member's ID document

  •   Proof of banking details ( a cancelled cheque or bank statement).
      The account must be in the members name.

  •   Fund membership proof ( an old benefit statement or salary slip will do)

Alexander Forbes  will need the original claim form before they can release any payment. Forms can be posted to:
