BENEFICIARY NOMINATION FORMS

 

It is important that you complete a Nomination of Beneficiary Form setting out details of your dependants and nominees.

 

You can either download a hard copy from the links below and submit it to Human Resources:

 

 OR  

You can also complete the form on AF Online. 

 

In order to access the online facility you will need a pin and password.  You can do this by following this link: https://online.alexanderforbes.co.za/Account/Login or calling 0860 100 333. The Alexander Forbes’ help desk will talk you through the requirements.  

                                                                                                                                        

Handy Hint: if you scan your ID document before you phone AFOnline help desk, they will talk you through the process and you should be online within 48 hours.

 

If you have already registered but cannot remember your login details, your PIN is your ID number. For a new password please press Change/Forgot Password and a new one will be sent to you.
